Any advice for someone starting the conversation about castration?

I am self neutered and have been for a number of years but I still have my balls. They’re completely nonfunctional, atrophied to the point they stay inside me most of the time, and withering. I started the conversation with my urologist today purely by accident.

She went over my recent ultrasound results which showed a possible lesion with dark fluid build up around my right ball. She opened up door by asking about testicular pain since my “incident” that caused a severe hydrocele a couple of months ago. Of course I jumped on it and said yes and told her they both ache considerably most days, the left had the same thing happen last year.

She seemed to be quite sympathetic with the situation and told me that with my permission she would send my results to the surgery team to look at and see if they thought the area of concern could be removed by cutting open the tunica and simply cutting and sliding it back. She said that if the testicle could be preserved they would do whatever possible if surgery happened.

I told her that if it had to go that was fine I was tired of dealing with the discomfort and that I was already on t replacement and had already had kids and we didn’t want more so I wasn’t concerned about anything as far as the other one stepping up to do the job of both, besides the left one is done for anyways and is the smaller of the two now.

So my question is how can I coax this along passively without raising any questions or concerns as to my motives when I tell her I’d prefer they just take both so I don’t have to have surgery twice if the same thing happens again?
